Medical Conditions Management


The Education Directorate is committed to providing a safe and healthy environment for students. While school staff have a duty of care to students to provide first aid assistance when required, parents need to be aware that schools cannot be responsible for the general management of medical conditions.

In special circumstances, staff may be able to assist with the administration of medication. In these cases, Education Department policy requires Principals to ensure that a comprehensive written authority is obtained from the student’s parents and also seek from them a written statement from the student’s doctor authorising a member of staff to administer the prescribed medication.

The following documents are to be completed annually.

Medical Information and Consent Form - all students need this form to be completed annually

Known Medical Condition Response Plan - for each medical condition

Medical Authorisation and Administration Record - long term medication

Medical Authorisation and Administration Record -  short term medication e.g. antibiotics

Information on a child's health status (e.g. epilepsy, diabetes) is appreciated at enrolment or upon diagnosis.

Music Program - Out of Hours


The Out-of-hours Music Program was initiated by a group of parents in 1989.

The program offers Fadden Primary School students the chance to learn a musical instrument after school hours, at the school. A sub-committee of the P&C Association employs professional teachers and offers classes in keyboard, recorder and guitar.
